    JOB DESCRIPTION – Nurse/LPN

    TITLE: Nurse LPN/RN FTE: 1.0

    FLSA STATUS: Non Exempt/Exempt HOURS PER DAY: 8 WORK CALENDAR: 198 days

    COMPENSATION: LPN Classified Non Exempt Compensation Guide (Starting at $26.70/hour)

    RN Classified Exempt Compensation Guide (Starting at 52,272)

    ESSENTIAL DUTIES AND RESPONSIBILITIES

    1. To provide health services for the assigned work site; to maintain appropriate, accurate and required student health records; and to coordinate and work with school nurses at other work sites

    2. Provides instruction, counseling, and guidance to students, parents, staff and others regarding health issues

    3. Upholds professional standards, the Nurse Practice Act, state and local statutes, and District policies and guidelines

    4. Maintains current health records as required by law on each student and compiles and files legally-required reports

    5. Establishes and maintains a comprehensive school health program

    6. Monitors immunization status of current and new students for compliance with law

    7. Obtains signed parental permits for routine and emergency care

    8. Collects information about the health and developmental status of students in a systematic and continuous manner

    9. Maintains files on current health status of students, on-going medical concerns, and known allergies, respecting students’ rights to privacy and confidentiality

    10. Provides appropriate nursing interventions and monitors student status as needed

    11. Provides for continuity of care upon transfer or graduation

    12. Coordinates service provision with IHS staff members and other sources of community health care as needed

    13. Provides or makes referrals for follow-up care

    14. Develops a nursing care plan with specific goals and interventions related to students with special needs

    15. Implements actions that promote or restore health and effect rehabilitation, assesses student responses and collaborates with other professionals to achieve optimal levels of wellness and learning

    16. Participates in IEP conferences when appropriate

    17. Provides staff education related to specific student needs

    18. Stores, logs, and dispenses over-the-counter and prescription medication to students

    19. According to physicians’ standing orders and the Nurse Practice Act

    20. Assists students, families and school personnel to achieve optimal levels of wellness through health education

    21. Maintains confidential emergency treatment information for staff members

    22. Provides ongoing monitoring of blood pressure, blood sugar, etc. as needed by staff and students, as well as appropriate education related to individual health needs

    23. Participates in the health curriculum committee

    24. Participates in in-service training programs

    25. Assists school personnel in maintaining sanitary standards in schools

    26. Participates in interviews, screenings and recommendation of nursing personnel for employment

    27. Supervises and trains new employees

    28. Performs Arizona state mandated health, vision and hearing screening, and maintains annual hearing machine calibration

    29. Completes annual Arizona state mandated immunization screening reports and hearing reports.

    30. Files reports of suspected abuse/neglect when initial contact

    31. Administers annual bloodborne pathogen inservice for all school staff

    32. Completes employee incident and workman’s compensation evaluations and assessments.

    33. Coordinates biohazard quarterly pick-up

    34. Budgets and orders medical supplies

    35. Maintain confidentiality of information regarding students, staff, parents and school issues

    36. Provide a welcoming atmosphere for students, staff, parents and community

    37. Perform responsibilities in an ethical and professional manner

    38. Maintain punctual and regular attendance

    39. Perform other related duties as assigned

    KNOWLEDGE, SKILLS AND ABILITIES REQUIRED

    To perform the job successfully, an individual must be able to perform each essential duty satisfactorily. The qualifications for this job description are representative of the knowledge, skill and/or ability required

    Education / Certificate

    1. Must possess Licensed Practical Nurse License.

    2. Must possess current Hearing Screening Certificate

    3. Must have and maintain a valid IVP Arizona Fingerprint Clearance Card

    Experience

    1. Recent experience in the fields of public health nursing, and/or school nursing is desirable.

    2. Must be 21 years of age or older upon hire.

    Communication Skills

    1. Bilingualism in Spanish is beneficial in this position

    2. Ability to communicate effectively with individuals and groups, both verbally and in writing

    Computer Skills

    1. Must possess a thorough knowledge in the use of office machines, computers, and Microsoft

    Office Publications (Google, Word, Excel, PowerPoint)

    2. Must have ability to use or to learn to use student information system software as well as other software necessary for the department
